ELIGIBLITY REQUIREMENTS

• Proof of responsible care
  • Current on vaccinations
  • Vet visit within past 12 months
• Proof of ownership of the pet
• Pet must be spayed or neutered
• Proof of financial hardship
  • Receives Medicaid, SNAP or Social Security
  • Unemployment
  • Written explanation of extenuating circumstances
• Diagnosis and itemized estimate from Vet
• Covers only emergency care and followup if needed
• Must have a positive prognosis

ITEMS NOT COVERED

• Cancer treatment, heart condition
• Spays or neuters
• Chronic conditions (ie; diabetes, cushings)
• Care already provided
• Elective or non-essential surgery
